Steps for payroll deduction registration:

  1. Complete this Request for Payroll Deductions for Professional Organization form

  2. Make one copy

  3. Send one copy back to NAVAPD at P.O. 15418, Arlington, Virginia 22215-0418

  4. Give one copy to your Payroll Services Office
